Types of Dermal Fillers and Their Lifespan
There are various types of dermal fillers, each with different compositions, effects, and durations:
Hyaluronic Acid (HA) Fillers
The most common type of filler, HA-based products such as Juvederm (both Hylacross and Vycross versions) and Restylane are popular for their natural-looking results and versatility. These fillers are traditionally thought to last between 6 to 18 months, depending on the specific product and the area treated. They dissolve gradually over time as the body naturally metabolizes the hyaluronic acid. Until July 2024, the public and physicians believed what the filler industry told us, which is that these fillers are generally fully dissolved by 6 to 24 months.
But recent news headlines call this claim into question, suggesting that HA fillers may be present many years later. This is not necessarily a bad thing, and has some benefits, including longer lasting support and some evidence of increased natural collagen formation. After all, if we invest and like our results, we don’t really want everything deflating every 6 months. But that said, it is evidence that there is all the more reason to consider the philosophy of “less is more” with fillers, and to be sure to see a thoughtful, artistic, conservative board-certified dermatologist or other “core” board-certified injector including only board-certified plastic surgeons, oculoplastic surgeons, and facial plastic surgeons for treatment. There is no downside to using small amounts and layering over time to get the best results. This also reduces the risk of filler ending up stuck in the wrong places over time.
Calcium Hydroxylapatite (CaHA) Fillers
Fillers such as Radiesse contain calcium hydroxylapatite, a mineral-like compound found in bones. CaHA fillers generally last longer than HA fillers, with results lasting up to 18 months. These fillers are often used for deeper wrinkles and for adding volume to the cheeks.
Poly-L-lactic Acid Fillers
Known for stimulating collagen production, poly-L-lactic acid fillers like Sculptra work over time to enhance the skin’s collagen. While the results are not immediate, they can last up to two years as the filler gradually dissolves while promoting natural collagen growth.
What Happens as Dermal Fillers Dissolve?
As dermal fillers slowly dissolve (if they do), the treated areas will gradually continue to age and may need more filler over time. The breakdown process is influenced by the type of filler, the individual’s metabolism, and the area of injection. Hyaluronic acid fillers, for instance, are naturally broken down by the body’s enzymes, particularly hyaluronidase. This process helps to allow some filler to reduce over time and allows changing priorities to be discussed with your physician as time passes.
Other types of fillers, like those made from poly-L-lactic acid or calcium hydroxylapatite, also dissolve but may have longer-lasting effects due to their unique properties. It’s important to note that the dissolving process is typically slow, resulting in a gradual reduction of the filler’s effects, which many people find preferable as it allows for subtle changes rather than abrupt shifts in appearance.
